10/7/2023 EnchantaVaughn Approved Upon arrival to the home setting, Josiah appeared to be in an ecstatic mood, as evidenced by his enthusiasm and positive expressions. Josiah was seen displaying appropriate social interactions in the session. Josiah maintains an excellent attitude, demonstrating a good rapport with QMHP-C. Josiah is observed practicing interventions without no complaints and having fun. QMHP-C prompted Josiah to express how he believes family members should treat each other. QMHP-C encouraged Josiah to think twice before reacting negatively or aggressively in situations that may not need that level of reaction to be solved. QMHP-C used the basic listening sequence to clearly draw out the point of view that Josiah was feeling and get beyond his aggressive language and behaviors. QMHP-C implemented individualized behavior management techniques with Josiah. QMHP-C processed with Josiah about psychoeducational activity while assisting with directions. QMHP-C explained the importance of the activity. QMHP-C provided feedback to Josiah in regard to questions asked. Josiah has made more of his own decisions, he reported increased satisfaction and function. Josiah responded well to feedback. Josiah agreed to behave and communicated a positive attitude when reviewing problem solving and self-control strategies. Josiah promised he would try his best to correct himself when faced with conflict. Josiah displayed respectful, non-aggressive behaviors in the home. All objectives were being met at this time, despite stressful environment. Josiah avoided distractions in social setting, while utilizing anger management skills. Josiah seem to be heavily influenced by others and pressured into doing things he shouldnât do. Josiah displays leadership skills by helping others and being good. Josiah acknowledged interventions and verbalized clear understanding of each intervention. Josiah engaged in reflective thinking and expressed his feelings to QMHP-C.
